NOTE I only recommend including that information if you are a brick & mortar physical store. DO NOT EVER share your personal phone number or home address publicly on the internet!]

Things to consider: 

Are you able to qualify potential clients via your form so you will already know if they will be a good fit for you? 

Service providers, do you want them to outline exactly what they are looking for, or are you going to ask something like, “which package were you interested in” with a dropdown menu? 

DEFINITELY—both product AND service providers!—include a part of the form that’s a bit open-ended of a question (check your survey questions from your Voice Hacking Workbook for ideas) … that’s a great way to get even more client voice hacking info. Saying something like “anything else you want me to know?” can work just fine. 

Do you need to do any price anchoring? Or are you asking their budget or asking them to pre-select from a menu? 

It can be helpful to add a “how did you hear about me” section, too!]
